JOB SUMMARY: The Inpatient Social Worker, in collaboration with the clinical team and medical provider, provides patient and family advocacy, discharge planning coordination, and psychosocial intervention for the high-risk inpatient. The Social Worker strives to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources among a patient population with complex health needs.

RESPONSIBILITIES:

• Conduct assessments of maternal and child health needs, to identify barriers to treatment and encourage compliance with medical care
• Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care for patients Create plan for care across the continuum, integrating patient/family preferences and values.
• Serve as a resource to assess, evaluate, recommend legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.
• Advocate for resources and removal of barriers
• Maintain ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
• Collaborates with healthcare providers, community organizations, and families to promote health education, preventive care, and effective health management strategies.
• Educate families about prenatal care, infant care, safe sleep, and Post Partum Mental Health
• Maintain accurate records of patient interactions and outcomes in compliance with hospital policies.
• Provide excellent communication skills with the ability to engage effectively with diverse populations.

REQUIREMENTS:

• Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline.
• Master's in Social Work required.
• 2 years of social work experience in a hospital setting with maternal child health, children and families is required.
• LMSW or LCSW/LCSW-C required.
• MD Social Work License per level of education.
• Demonstrates the ability to follow verbal instructions, as well as the ability to communicate effectively both verbally and in writing.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. 
• Case Manager credentialed from the American Case Management Association (ACMA) preferred.
